IV vitamin C (high-dose vitamin C drip): who is it for and how is it given?

Short answer: Vitamin C given into a vein reaches blood levels that cannot be achieved by taking it by mouth. G6PD and kidney tests are requested before treatment; in people with diabetes, finger-prick glucose readings may be misleading. Vitamin C does not prevent colds and is not a substitute for vaccination.

Vitamin C (ascorbic acid) is a vitamin the body cannot make itself, so it must come from outside; it plays a role in immune cell function, collagen production and iron absorption. Most of the daily requirement can be met with a balanced diet; in some situations, however, a doctor may consider giving vitamin C intravenously.

What is the difference between IV vitamin C and taking it by mouth?

When vitamin C is taken by mouth, absorption from the gut falls as the dose rises, so blood levels do not go above a certain limit. When it is given into a vein, much higher blood levels can be reached within a short time. The health effects of these high levels are still being studied, and results vary from person to person.

When is it considered?

IV vitamin C may be considered for people found to have vitamin C deficiency, for complaints of marked tiredness and fatigue, and as an option to support immunity in situations the doctor considers appropriate. A note: studies show that regular oral vitamin C does not prevent colds in the general population and may only slightly shorten their duration. For this reason, vitamin C is not a substitute for vaccination and other preventive measures.

Which tests are needed before treatment?

  • G6PD enzyme level: in people with G6PD deficiency, high-dose vitamin C can cause the breakdown of red blood cells (haemolysis). This test is therefore requested before high-dose treatment.
  • Kidney function tests: vitamin C is excreted by the kidneys, so it is important to know how well the kidneys are working.
  • A full blood count and other basic tests if needed.

Who should not have it, or needs extra caution?

  • People with G6PD deficiency
  • People with kidney failure or on dialysis
  • People with a history of kidney stones (especially oxalate stones)
  • People with iron overload disease (haemochromatosis)
  • Pregnant women (not given without a doctor's assessment)

Important warning for people with diabetes

High-dose IV vitamin C can make some finger-prick glucose meters show blood sugar as higher than it really is. Because this could lead to an incorrect insulin dose, people with diabetes must tell us before treatment and plan their glucose monitoring on the day of treatment with their doctor.

How is it given?

The dose is set by the doctor according to your weight, test results and the purpose of treatment. The drip is given slowly into a vein; depending on the dose, it usually takes 30–90 minutes. Drinking plenty of water during the session is recommended. A mild burning feeling at the drip site, thirst or temporary dizziness may occur.
