Crystallized phenol treatment for pilonidal sinus: what is it and how is it applied?

Short answer: Crystallized phenol is a treatment option for suitable pilonidal sinus patients, done under local anaesthesia without a large incision or stitches. The tracts under the skin are cleaned of hair and debris, then crystallized phenol is applied in a controlled way to help them heal and close. Patients usually go home the same day. One session is enough for some, others need more, and the disease can recur. Op. Dr. Salim Balin performs it at his clinic in Ataşehir, Istanbul, after consultation in suitable patients.

What is crystallized phenol treatment?

Crystallized phenol treatment can be used in suitable pilonidal sinus patients without a large incision or stitches. The aim is to clean the tracts (sinuses) under the skin of hair and debris and then help them heal and close through the effect of phenol.

Pilonidal disease forms when hairs enter the skin over the tailbone, creating tracts and inflammation. Phenol acts chemically on diseased tissue in the tract; laser closure pursues the same goal with energy. See phenol treatment and laser treatment.

How is crystallized phenol applied?

Before treatment, the extent of disease, tract structure and any abscess are assessed. In suitable patients it can be done in clinic under local anaesthesia.

  1. Numbing: the area is numbed with local anaesthesia.
  2. Cleaning: hair and debris in the tracts are carefully removed.
  3. Skin protection: surrounding skin is protected, as phenol on healthy skin can cause a chemical burn.
  4. Phenol: crystallized phenol is applied into the tracts in a controlled way to support healing and closure.
  5. Dressing: a dressing is applied and care advice given.

Can you return to daily life after crystallized phenol?

Patients usually go home the same day. Return to activities and work is often possible soon, depending on extent and comfort. Local anaesthesia controls pain during the procedure; mild pain, tenderness or discharge may follow.

Follow your doctor’s advice on bathing, dressings and hygiene. Early return to daily life does not mean the tracts have fully healed; follow-up visits matter. Seek care for increasing pain, redness, fever or foul-smelling discharge.

Is one session enough?

For some patients one application is enough; others need more. Success depends on disease type, tract cleaning, technique and follow-up, and the disease can recur.

Keeping the area clean and dry, controlling hair and losing excess weight help reduce recurrence.

Can crystallized phenol be used for every patient?

It is not suitable for everyone.

  • With an acute abscess, drainage may come first; phenol is considered once inflammation settles.
  • In extensive, multiple-tract or recurrent disease, laser or surgery are also considered.
  • Discharge near the anus should be assessed for other causes such as anal fistula.

Because phenol on healthy skin can cause a chemical burn, it should be applied by a doctor with protective measures.

What are the success and recurrence rates?

A 2024 retrospective study from Türkiye of 77 patients reported about 75% success after the first application and 96% after one or two sessions, with about 4% recurrence at one year. Higher BMI, heavy hair growth and infrequent shaving were linked to recurrence.

A small 2025 study in adolescents reported 75% complete healing, with some skin burns and cellulitis. These are single-centre retrospective studies, and results vary with disease and technique.

A review of published series on phenol treatment reported an average success rate of about 87% and an average return to work of 2–3 days, with better results in patients with 1–3 sinus openings. However, few randomised trials exist, so the evidence is limited and this is not a promise of individual results (Kayaalp and Aydın, Tech Coloproctol, 2009).
